As our Canadian healthcare system focuses on transitioning patients from hospital to home, the dietitian's role in continuing care for malnourished patients will be even more important. In this presentation you will learn how two organizations are working together with community partners to tackle malnutrition across the continuum of care. We will hear from one of the hospitals involved in the More2Eat study and another with perspective on continuing nutrition care in the rural setting. The session will focus on their experiences of creating new processes, resources and building links with their communities as malnourished patients return home.
About the speakers:
Marilee Stickles-White, BASc, RD graduated from the University of Guelph and interned with the Ottawa Regional Dietetic Internship Program. She has been working at Niagara Health System for more than 20 years in progressive clinical nutrition and leadership positions. She is a member of the DC Clinical Nutrition Leaders Action Group. Marilee has developed a strong interest in addressing the need for improved identification and treatment of malnutrition in acute care stemming from participation in the Nutrition Care in Hospitals study. She was the Site Champion for the More-2-Eat Study representing Niagara Health System and continues to work on spreading the use of INPAC throughout her organization.
Amber Merrell, RD works for Alberta Health Services. Amber graduated from Acadia University with a Bachelor of Science in Nutrition with Co Operative Education and completed the Acadia Dietetic Internship Program. After internship, Amber returned to her home province to start her career and has worked at the Healthcare Centre in Barrhead, Alberta for the past 4 years. Amber is involved with local initiatives to improve collaborative and person-centred care in-hospital and she enjoys using an adaptive skillset to work in many areas of dietetics as a rural dietitian.
